Redditch and Bromsgrove Primary Care Trust has injected £360,000 into the service to provide a consultant, two nurses and an occupational therapist over the next 12 months.
The development will be specifically for Redditch and will replace the existing Outreach service provided from Bromsgrove.
The PCT, which came into existence in April following the termination of Worcestershire Health Authority, has an annual budget of £110 million.
PCT spokesman Kimaria Sharpe said the aim of the organisation was to improve all health services in Redditch.
A guide to all health services in Redditch is also being produced and will be available to townsfolk in October.
