Healthwatch Worcestershire has called for dementia patients and carers to share experiences to improve NHS and social care services.
The campaign has asked people diagnosed and their unpaid caregivers to share their post-diagnosis experiences.
John Taylor, director at Healthwatch Worcestershire, said: "We need to hear from those people diagnosed with dementia to fully understand their experiences of information and support."
The collected feedback will shape future improvements in dementia services in the county.
Surveys can be filled in online, over the phone, or requested as a paper copy.
